The above puppy has been examined by a licensed veterinarian and has been
found to be free of defects and is current on all vaccines and deworming for his/her
age. We strongly recommended that the owner/buyer finish the complete series of
vaccines to protect your puppy against sickness and disease. Also it is highly
recommended that you start your new on a monthly heartworm preventive. Please
take the following with you to your new puppy exam: Vaccine & deworming
records and notes from the seller/breeder's licensed veterinarian.
Above puppy is a pet quaility puppy with limited registration. The owner/buyer
agrees to have above puppy examined by a licensed veterinarian within a 72 business
hour time period from time of purchase or arrival.
After having having your new puppy examined by a licensed veterinarian
owner/buyer agrees to send a copy of the veterunarians exam notes by registered mail
signature requested.
If owner/buyer fails to have puppy examined and or does not provide breeder/seller
with a copy of the veterinarians notes this contract will be
completly null and void.
After expiration of the 72 hour guarentee a one-year congenital health guarentee
applies as long as the buyer/seller complies with the above agreement. Congenital
means a defect that the above puppy was born with but has gone undetected.
Owner/buyer agrees to contact seller/breeder at once and have the above puppy
examined by a second licensed veterinarian chosen by the breeder/seller at the
breeders expense within the 1 year guarentee.
If two licensed veterinarians within two different practices agree that the defect will
greatly affect the above puppys quality of life or cause premature death. Seller/breeder
will replace the above puppy with one of equal quality,color and sex if that is what the
owner/buyer requests as soon as one becomes available .
If above puppy expires dueto the defect owner/buyer agrees to have the defective
puppy properly shipped or brought back to seller/breeder for necropsy within a 24
hour period.
Defective puppy, signed veterinarian statement of the congenital defect from
owner/buyers veterinarian and registration papers, included but not limited to properly
executed registration papers. Transfer and late fees, (if any) applies required by the
issuing registry, must be returned to sell/breeder at the owner/buyer expense
before puppy will be replaced.
Seller/breeder will refund the price of the puppy only. Shipping fees will not be
refunded if any and theowner/buyer will pay any additional shipping fees if applies.
Seller/breeder will not be responsible for any medical services preformed by a
licensed veterinarian, not authorized by the seller/breeder at any time. Guarentee does
not cover the following patellas, bite, sizecolor, weight, parasites, heartworms ,
umbilical hernias.
Seller/breeder has the right to first refusal if the owner/buyer is unable to keep the
above puppy for any reason.
If the seller/breeder does not wish to take puppy back, the seller/breeder will assist
in finding a suitable home. It is agreed that the above puppy will not be turned over to
a shelter, rescue or any other enterprise for any reason or excuse.
Owner/buyer certifies that they are not licensed as a USDA licenscee, nor are they
acting on behalf of a pet store, or puppy broker. That the above puppy is purchased for
their own and not for resale.
No other warrantees, expressed or implied are made under this contract except
stated herein. We further agree that any claims that might arise from the sale will not
exceed the purchase price of the above puppy only (shipping not included). A written
statement by the veterinarian must be sent along with claim.
Having read and fully understanding of the contents herein, I/we cheerfully enter
into this agreement.
